Complete this form and send to Specialized Loan Servicing LLC (“SLS”).  To help expedite the process, log into your online account: click on Offers in the top right of the screen.  Once this loads, click on Let’s Get Started towards the bottom of the screen and complete the online interview.

Once you have completed the Mortgage Assistance Application, you may send the documents by mail to:
