Guest: You want to replace a lost certificate
Penny:If your share certificate(s) has been lost, misplaced or stolen, please notify Computershare by calling our National Customer Contact Center (click here to retrieve the appropriate toll-free number). A customer service representative will place a preventative stop on the missing certificates and mail you the required documentation for replacement.

Alternatively, you may complete and send us the “Account Update / Inquiry” form available in our Printable Forms section. Please indicate the following information in the “other” section of the form:

• the name and type of securities you own
• certificate number(s) (if known)
• the reason for the loss (i.e. lost, stolen, destroyed, or never received)

Upon receipt of your notification, our records will be updated within 24 hours with a “stop transfer notation” for each certificate number provided.
